Acute Fulminant Hepatitis following Operation



Abstract
The change of the hepatic unction after operation is usually mild and reversible but they may be fatal if the patient had already septic shock, massive blood transfusion, previous hepatic disease, induction of microsomal enzyme system, hypoxia and hypotension during anesthesia.
Halothane is most easily blamed when a post-operative case of the hepatitis occurs because it is most avidly used as an anesthetic agent and the hepatitis has been intermittently reported for a long time. However, it is not easy to prove halothane-induced the hepatitis clinically.
We experienced a case of fulminant hepatitis following operation. High fever and jaundice with the marked elevation of SGOT, SGPT and bihrubm were appeared on 11th postoperative day in the patient of 28-year old female scheduled for retinal detachment under halothane anesthesia.
Several factors such as blood transfusion, exposure ¢¥of the unrecognized hepatotoadc drugs, enzyme inducing drug and deviation of genetic or immune system might be attributed to development of the acute fulminant hepatitis.
